In the original novel, the flashback scenes of elderly Paul in the nursing home are more brief. The film extends these segments and adds details where Paul admits he is 108 years old and has lived so long due to Coffey's "stream of life," reinforcing the continuity of the tragedy.
The film cut the role of Paul's wife, Jane, from the original novel, keeping her only as a background character. Meanwhile, the prison guard Dean Stanton has a milder personality in the film, whereas in the novel he is more aggressive.
In the original novel, after Coffey healed Paul, Paul briefly gained Coffey's "empathy," seeing visions of the world's evil. However, the film omitted this supernatural experience, only retaining the scene where Coffey transfers pain.
